Quadricuspid Aortic Valve Defined by Echocardiography and Cardiac Computed Tomography

چکیده
A 54 year old female presented with lower extremity edema, fatigue, and shortness of breath with physical findings indicative of advanced aortic insufficiency. Echocardiography showed severe aortic regurgitation and a probable quadricuspid aortic valve. In anticipation of aortic valve replacement, cardiac computed tomography (Cardiac CT) was performed using 100 kV, 420 mA which resulted in 6 mSv of radiation exposure. Advanced computing algorithmic software was performed with a non-linear interpolation to estimate potential physiological movement. Surgical photographs and in-vitro anatomic pathology exam reveal the accuracy and precision that preoperative Cardiac CT provided in this rare case of a quadricuspid aortic valve. While there have been isolated reports of quadricuspid diagnosis with Cardiac CT, we report the correlation between echocardiography, Cardiac CT, and similar appearance at surgery with confirmed pathology and interesting post-processed rendered images. Cardiac CT may be an alternative to invasive coronary angiography for non-coronary cardiothoracic surgery with the advantage of providing detailed morphological dynamic imaging and the ability to define the coronary arteries non-invasively. The reduced noise and striking depiction of the valve motion with advanced algorithms will require validation studies to determine its role.

Cardiac computed tomography and quadricuspid aortic valve: a case report.
A quadricuspid aortic valve is rare and often incidentally found by echocardiography, surgically, or on post mortem examination. Aortic regurgitation is common and if severe enough can lead to symptoms of dyspnea. We report a case of a quadricuspid aortic valve, which was found by cardiac multidetector computed tomography during a pre-operative assessment for severe aortic regurgitation.

OBJECTIVE The aim of this study was to identify the morphological and functional characteristics of quadricuspid aortic valves (QAV) on cardiac computed tomography (CCT). MATERIALS AND METHODS We retrospectively enrolled 11 patients with QAV. All patients underwent CCT and transthoracic echocardiography (TTE), and 7 patients underwent cardiovascular magnetic resonance (CMR). The presence and ...

CASE SUMMARY A 10-month-old male domestic shorthair cat was brought to Kitanomori Animal Hospital for routine castration. Preoperative thoracic radiography revealed a mildly enlarged heart silhouette, and transthoracic echocardiography (ECHO) suggested a quadricuspid aortic valve associated with central aortic regurgitation (regurgitant fraction 31%). After sedation with intramuscular medetomid...
متن کاملQuadricuspid Aortic Valve Complicated with Severe Aortic Regurgitation and Left-Sided Inferior Vena Cava
A 72-year-old man with hypertension was referred to our hospital for severe aortic regurgitation probably associated with quadricuspid aortic valve on transthoracic echocardiogra-phy. He felt general fatigue on effort. On physical examination , blood pressure was 166/54 mm Hg and cardiac ausculta-tion indicated a systolic murmur at the aortic area together with a diastolic murmur at the left pa...
متن کاملQuadricuspid pulmonary valve in an adult patient identified by transthoracic echocardiography and multi-detector computed tomography.
Quadricuspid pulmonary valve is a rare congenital heart disease. It is infrequently associated with significant clinical complications and tends to be clinically silent. Because of its benign nature, it has been diagnosed mainly post mortem. Its diagnosis by transthoracic echocardiography is very difficult because of the anatomical features. We describe a case of quadricuspid pulmonary valve di...
